Improvements announced for remote chronic-disease monitoring in Ontario
The Ontario Telemedicine Network (OTN), which has been providing remote access to healthcare and education for more than a decade, is partnering with digital health platform vendor Vivify Health to enhance its Telehomecare program.

Launched as a pilot in 2007, OTN Telehomecare is a remote patient monitoring program focused on those with chronic obstructive pulmonary disease (COPD) and/or congestive heart failure (CHF), which are two of the top three chronic conditions that result in hospitalization. Through the program specially trained nurses monitor patient data daily, as well as coaching patients to help them manage their condition for improved quality of life.

The program is provided by nine of Ontario’s 14 local health integration networks (LHINs). Supporting the province’s “patients first” agenda, Telehomecare has been shown to reduce hospital admissions and ER use among this patient population by over 60%. A recent OTN patient survey found over 98% of patients were satisfied with the program and 95% with an improved ability to self-manage their condition.

Through an open procurement process, Vivify was selected as the exclusive vendor to help meet the needs of the evolving Telehomecare program. The firm uses cloud services that utilize consumers’ mobile devices, as well as other means to monitor and engage health consumers remotely.

Vivify’s platform includes features such as videoconferencing, customized protocols and guided pathways, support for multiple languages, non-stop technical support, and interactive voice response (IVR) solutions.
